The diocese was formed in the year 2006 on the basis of the recommendation of the regional Holy Episcopal Synod to the Holy Throne of Antioch. The decision was to separate the Parishes in the Kannur, Kozhikode and Malappuram districts which were erstwhile part of the Diocese of Malabar .
From 2006 to 2008, the diocese was under the leadership of L.L Thanono Mor Philexinos Yuhanon,the then Metropolitan of the Diocese of Malabar. Again the diocese was under H.B BaselioseThomas I Catholicose for a brief period of time in 2008. Thereafter, Rev.Fr Paulose Vettikunel became the Metropolitan elect for the Diocese of Kozhikode and was consecrated by L.L Patriarch H.H Morn Mor Ignatius Zakka I as Paulose Mor Ireneous at the Patriarchal Cathedral, Bab Touma ,Damascus, Syria on 24th August 2008.
